What the coordinator does

    Open in patient context

    The app launches from the chart with demographics, address and coverage already populated.

  2. 02

    Pick the protocol

    CHF 72-hour, COPD, falls assessment, behavioral health outreach, treat-in-place follow-up and more.

  3. 03

    Set the window and escalation

    Choose the visit window and the escalation rule the program should follow on findings.

  4. 04

    Send and move on

    The mission broadcasts. Status returns to the app and to the coordinator's work queue view.

Data in

Read-only FHIR resources scoped to the launch context: Patient, Coverage, Condition, Encounter and Practitioner. Only what the mission needs.

Data out

Mission status and outcome summary are available in the app view and over the API. DispatchCare does not promise clinical write-back to the chart.

The Epic deployment is one instance of the same app. SoFaaS, VectorCare's SMART on FHIR as a Service layer, builds DispatchCare once and carries it into Epic, Oracle Health, MEDITECH, athenahealth and other charts, so an Epic hospital and a MEDITECH critical access hospital in the same region run the same referral workflow and feed the same program-level reporting.

VectorCare's patient-logistics applications have been live in Epic since 2025, which is why the Epic path is the shortest one on this site: the framework, the authorization pattern and the audit posture are all already in production.

No EHR IT project required to start

Hospitals on Oracle Health, MEDITECH or anything else deploy through SoFaaS or use the secure web referral form from day one, and can move into the chart later.
