What is patient logistics?
Patient logistics is the coordination of the non-clinical services a care plan depends on: mobility, durable medical equipment, home health, deliveries and home visits.
Every discharge plan and home-based care plan carries logistics. Somebody must arrange the equipment, the home health referral, the delivery and the visit, and confirm each one happened.
Patient logistics infrastructure is the shared rail those requests travel on, so a need surfaced during a home visit becomes a tracked child request rather than a second fax.
