What is community paramedicine?

Community paramedicine (CP) extends the paramedic scope beyond emergency response into planned home visits: post-discharge checks, chronic disease monitoring, medication reconciliation, fall assessments, behavioral health follow-up and treat-in-place encounters. Programs are run by EMS agencies, fire departments, hospital-based EMS services and private ambulance companies, and operate under an approved scope and medical direction that varies by state.

The distinguishing feature is that the visit is scheduled and preventive rather than dispatched and emergent. A CP program is measured on whether patients avoided a return to the hospital, not on response times.

Coordination is the persistent weak point. Referrals arrive by fax and phone, status rarely returns to the referring hospital, and the sponsor paying for the program often sees nothing but a count of visits. DispatchCare exists to make the request, the status and the proof a single record.
