What is treat in place?
Treat in place is an EMS pathway where a patient is assessed and treated at home, with clinician consultation, instead of being transferred to an emergency department.
Treat in place pairs an on-scene EMS assessment with online medical control, a physician or advanced practice clinician reached by phone or video, to reach a documented disposition: remain at home with a plan, go to a clinic or alternate destination, or transfer.
The model matters most where the nearest emergency department is far away, because the transfer itself carries cost, risk and hours of unit time. CMS language in state Rural Health Transformation plans uses the term directly.
A defensible treat-in-place program records the clinician on the call, the disposition and what happened in the following 72 hours. Programs that report only avoided transfers do not survive review.
