What is rural health transformation program (rht)?
The Rural Health Transformation Program is a $50 billion CMS program running FY2026 through FY2030 that distributes funds to states, which award them to rural subrecipients.
CMS announced first-year awards on December 29, 2025 to all 50 states, with per-state awards reported between $147 million and $281 million. Half of the annual pool is split equally across approved states and half is awarded on merit against state plans.
Funds flow from CMS to the state, and from the state to subrecipients, hospitals, EMS agencies, FQHCs and others, through subawards governed by 2 CFR 200 Uniform Guidance. Vendors and subcontractors are paid by subrecipients; they do not receive RHT funds directly.
States file three quarterly progress reports and one annual progress report, plus an annual financial report, so reporting obligations flow down to every subrecipient.
